Clinic Terms and Conditions – TC01

Definitions 

For the purposes of these Terms and Conditions: 

• “Clinic” refers to FYC Aesthetics Ltd trading as Forever Young Cosmetic.

• “Healthcare professional” refers to an appropriately qualified and registered practitioner working within their professional scope of practice and competence, including NMC-registered nurses and NMC-registered nurse independent prescribers.

• “Treatment” refers to any consultation, assessment, procedure, intervention, prescription, or course of care provided by the clinic, whether invasive or non-invasive, medical or aesthetic in nature. This includes all injectable treatments, prescription-only medicines (POMs), regenerative and skin therapies (including topical and chemical skin treatments), device- or energy-based treatments, and any associated advice, aftercare, review, or follow-up provided by the clinic, whether currently offered or introduced in the future. 

• “Patient” refers to any individual attending the clinic for consultation and/or treatment. 


1. Booking Fee and Acceptance of Terms 

1.1 A £30 booking fee is required to secure all appointments. This fee contributes towards the cost of a consultation with an appropriately qualified and registered healthcare professional and covers reserved clinic time, clinical assessment, record keeping, and pre-treatment advice. 

1.2 Where treatment proceeds at the scheduled appointment, the booking fee will be deducted from the final treatment cost. Where treatment does not proceed, the booking fee is retained to cover consultation time and is non-refundable. 

1.3 By booking an appointment online, via the Forever Young Cosmetic booking system, or directly with a member of staff, you confirm that you have read, understood, and agree to be legally bound by these Terms and Conditions, including the cancellation and rescheduling policy. 


2. Eligibility and Age Restrictions 

2.1 Forever Young Cosmetic does not book appointments for, consult with, or provide treatments to any individual under the age of 18 years. 

2.2 If a patient appears to be under the age of 25 years, valid photographic identification (passport or driving licence) will be required prior to treatment. 

2.3 Children and guests are not permitted in treatment rooms due to safety, safeguarding, and insurance requirements. One chaperone may be permitted upon request and at the discretion of the clinic. 


3. Appointments 

3.1 Appointments are advised to be booked in advance to secure preferred dates and times. 

3.2 Appointments may be booked online or directly with a member of the Forever Young Cosmetic team. 

3.3 The £30 booking fee is required to secure all appointments and will be deducted from the final treatment cost at the scheduled appointment. 

3.4 If, following clinical assessment, the healthcare professional determines that treatment is not appropriate or does not proceed for clinical or safety reasons, the booking fee will be retained to cover the cost of the consultation. 


4. Booking Confirmations 

4.1 Upon booking, confirmation will be sent via email and/or SMS.
4.2 A reminder will be sent approximately 24 hours prior to the scheduled appointment. 

4.3 If confirmation is not received, patients must check spam or junk folders and contact the clinic for confirmation to be resent. 


5. Arrival Time and Lateness 

5.1 Patients are requested to arrive at least 5 minutes prior to their scheduled appointment to allow time for paperwork. 

5.2 Appointments may be cancelled where a patient arrives more than 5 minutes late and the lateness materially affects safe delivery of care or clinic scheduling. 

5.3 Late arrival may result in forfeiture of the £30 booking fee, and a new £30 booking fee may be required to book another appointment. 

5.4 Any individual who is late on more than two occasions may be refused future appointments at the clinic. 

5.5 Consultations and treatments will not be rushed under any circumstances. 


6. Consultations 

6.1 An initial consultation is mandatory for all new patients and will be conducted by an appropriately qualified and registered healthcare professional. 

6.2 Consultations and treatments may be carried out by: - An NMC-registered Nurse Independent Prescriber (V300), or - An NMC-registered nurse acting within their professional scope of practice and competence, under appropriate clinical governance arrangements. 

6.3 Appointments are booked via the clinic’s booking system, which allows patients to select their preferred practitioner. The practitioner’s role and registration status are displayed at the point of booking. 

6.4 A full medical history will be taken. Patients must provide accurate and complete information regarding medical conditions, treatments, medications, and allergies. 

6.5 Clinical photographs may be taken, and all relevant consent forms must be completed prior to treatment. 

6.6 Failure to disclose relevant medical information may result in refusal of treatment. 


7. Treatment Suitability and Refusal 

7.1 All treatments are subject to clinical assessment and professional judgement. 

7.2 The clinic reserves the right to refuse treatment where it is deemed clinically unsuitable, unsafe, outside scope of practice, or not in the patient’s best interests, in line with professional standards. 

7.3 Treatment may also be refused where patient expectations are unrealistic or disproportionate, where outcomes cannot be reasonably achieved or guaranteed, or where proceeding would not be clinically appropriate. 


8. Results and Aftercare 

8.1 Individual results may vary and no specific outcomes can be guaranteed. 

8.2 Patients must follow all post-treatment aftercare instructions provided. Failure to do so may increase the risk of complications and affect results. 


9. Payments and Pricing 

9.1 Payment is due at the time of service unless otherwise agreed.

9.2 The clinic accepts cash, bank transfer, Clear Pay, Apple Pay, Google Pay, Samsung Pay, and all major debit and credit cards. 

9.3 Prices are subject to change without prior notice. Prices applicable are those confirmed at the time of booking or consultation. 


10. Cancellation and Rescheduling Policy 

10.1 A minimum of seven days’ notice is required to reschedule an appointment without forfeiting the booking fee. 

10.2 Appointments may only be rescheduled once within a 3-month period without incurring an additional charge. 

10.3 Cancellations within 48 hours of the scheduled appointment may incur a charge of up to 50% of the planned treatment cost. 

10.4 Cancellations on the day of the scheduled appointment may incur a charge of up to 80% of the planned treatment cost. 

10.5 Failure to attend without notice (no-show) may incur a charge of up to 100% of the planned treatment cost. 

10.6 Any cancellation or non-attendance charges will be applied proportionately and reasonably, will not exceed the clinic’s reasonable losses, and will take into account the circumstances of the cancellation. Where the clinic is able to reallocate the appointment slot, any cancellation charge will be reduced accordingly. 


11. Refunds 

11.1 Except where required by law, services rendered are non-refundable due to the nature of medical aesthetic treatments. This does not affect your statutory rights. 

11.2 Product refunds or exchanges may be considered only under specific circumstances and at the discretion of the clinic. 


12. Gift Certificates 

12.1 Gift certificates are available for purchase and are subject to expiry dates and specific terms of use. 

12.2 Gift certificates are non-refundable and cannot be exchanged for cash. 


13. Patient Information, Privacy and Confidentiality 

13.1 All patient information is treated as confidential and processed in accordance with the Data Protection Act 2018 and UK GDPR. 

13.2 Patient records and clinical photographs are stored securely on password-protected systems, including Aesthetic Nurse Software and secure external storage systems. 

13.3 Email addresses may be added to the Forever Young Cosmetic mailing list, with the option to unsubscribe at any time. 

13.4 Clinical photographs may be taken for medical documentation and continuity of care. Images will not be used for marketing or promotional purposes without separate, explicit written consent. Refusal of marketing consent will not affect eligibility for treatment. 

14. Personal Belongings 

14.1 Patients are responsible for their personal belongings. The clinic accepts no liability for lost, stolen, or damaged items. 


15. Patient Conduct 

15.1 The clinic operates a zero-tolerance policy toward abusive, aggressive, discriminatory, or inappropriate behaviour toward staff or other patients. 

15.2 The clinic reserves the right to terminate appointments and refuse future treatment where such behaviour occurs, without refund. 


16. Clinic Cancellations and Events Outside Our Control 

16.1 The clinic shall not be liable for cancellations or delays caused by events beyond its reasonable control, including but not limited to illness of clinical staff, emergencies, adverse weather, power failure, system outages, regulatory requirements, or public health guidance. 

16.2 In such circumstances, appointments will be rescheduled at the earliest reasonable opportunity. No additional compensation shall be payable. 



17. Continuity of Care 

17.1 The clinic does not guarantee the availability of any specific practitioner, treatment, or pricing structure beyond the date of service. 

17.2 Ongoing or future treatments remain subject to clinical assessment, practitioner availability, and prevailing prices at the time of booking. 

17.3 The clinic provides patients with an emergency contact number for post-treatment concerns or complications. This number is intended for guidance and support only and is not continuously monitored. 

17.4 Patients are advised that if they are unable to make contact via the emergency number, or if they experience severe, worsening, or urgent symptoms, they must seek immediate medical attention via their GP, NHS 111, or emergency services. Patients should not delay seeking urgent care while attempting to contact the clinic. 


18. Third-Party Advice and Information 

18.1 The clinic accepts no responsibility for advice, information, or guidance obtained from third parties, including social media, online sources, or other practitioners. 

18.2 Treatment decisions are based solely on the clinic’s own clinical assessment and professional judgement.

 

19. Complaints 

19.1 Any concerns or complaints should be reported to the clinic within a reasonable timeframe to allow proper investigation. 

19.2 Further details are available in the Forever Young Cosmetic Complaints Policy. 

19.3 Clinical practice within the clinic is supported by internal policies and procedures, including but not limited to consent, safeguarding, infection control, medicines management, record keeping, and adverse event and complication management. 

20. Right to Modify Terms 

20.1 Forever Young Cosmetic reserves the right to amend or update these terms and conditions. Reasonable notice will be provided where possible. 


21. Governing Law 

21.1 These terms and conditions are governed by and construed in accordance with the laws of England and Wales.
